Grapevine Relief and Community Exchange (GRACE)
GRACE offers the following services:
- Financial Assistance: GRACE provides emergency assistance to more than 1,200 families per year who are faced with financial challenges due to low-income or recent emergency. Aid is given on an as-needed basis. This program provides food, clothing, toiletries, household goods, furniture, financial assistance, and information/referrals to those in need. Monthly supplemental food is offered for seniors and the disabled.
- Medical Clinic: The GRACE Community Clinic is a Medical Home for uninsured families that do not have access to traditional health care. A dedicated staff of volunteer Physicians, Nurse Practitioners, Physician Assistants, Nurses, Pharmacists, translators and clerical personnel provide services to our patients. The clinic offers the following healthcare services: routine Family Practice, Internal Medicine, Endocrinology, Cardiology, Well Woman, and Chiropractic Care.
- Transitional Housing Program: The GRACE Transitional Housing program works with homeless individuals and families to help move them towards independence. Transitional Housing clients participate in intensive case management to assist them in achieving educational, career and financial goals while living in clean, stable housing. All activities are aimed at self-determination and eventual self-sufficiency. Transitional Housing case managers use a number of measures to track progress towards financial, educational and behavioral modification goals. Case management appointments are scheduled at least weekly and case managers assist with day-to-day functioning. Apartments used as housing for TH participants are completely furnished with donated furniture and housewares. TH participants can be assisted through other GRACE programs including the GRACE Pantry, Clothing Room and/or the GRACE resale stores. Medical needs are cared for at the GRACE Community Clinic and additional needs such as transportation costs, tuition aid and school supplies, daycare and other expenses can be partially funded by GRACE or through referrals/collaborations with other service agencies or churches.
Visitors are seen on a walk-in basis between the hours of 9:00am and 5:00pm on Monday, Tuesday, Thursday, and Friday, and between 12:30pm and 5:30pm on Wednesday. GRACE Emergency Assistance and Food Pantry also will be open the second Saturday of every month between the hours of 9:30am and 12:30pm.
GRACE serves residents of Grapevine, Colleyville, and Southlake who meet the income requirements below.
- General assistance = 125% of the Federal Poverty Guidelines
- General assistance for senior citizens = 185% of the Federal Poverty Guidelines
- Medical assistance = 200% of the Federal Poverty Guidelines
When you visit GRACE, please bring the following items with you.
- Paycheck Stubs (for the previous 30 days) for all working adults in the household
- Current Bank Statement
- Copies of all Financial Assistance (Food Stamps, TANF, Child Support, SSI, Social Security, Unemployment, Worker’s Comp, Disability, Retirement)
- Drivers license OR photo ID for all adults in the household
- Birth Certificate or Social Security card for each adult
- Birth Certificate for each child in the household
- Copy of Current Lease (complete) signed by tenant and landlord OR Residence Verification Form which is available in the office at GRACE
- Current school registration or report cards for all school age eligible children
