Prevent Blindness Texas

Prevent Blindness Texas (PBT) offers vision assistance to both children and adults through the following programs:

• Sight for Students:  This program provides FREE EYE EXAMS AND EYEGLASSES , when prescribed, to those children who qualify.  To apply please go to the website and print the “Sight for Students Program Application Form”.  You can take in person or mail the application to the Houston office which is included on the application.
• Healthy Eyes Eyeglass Program:  This program provides eyeglasses to adults who qualify.  To apply please go to the website and print the “Healthy Eyes Eyeglass Program Client Application Form”.  Along with the application you must send a current eye prescription (less than one year old).  You can submit the application via fax or mail.  The office address and fax # are included on the application.
• Resource Guide:  You can access the resource directory by going to the website and clicking on “Programs and Resources” then “Prevent Blindness Texas Vision Service Resource Directory”.
