Quality Support Coordination

Quality Support Coordination Inc will recognize the needs of an individual with a disability, develop support to enable the individual to live in the community and achieve desired personal choices while maintaining the participant’s health and safety.

Support Coordinator links the individual to supports to purchase their own home as well as coordinating and assisting individuals to access employment opportunities, volunteer experiences, roommates, transportation, family/ individual counseling, equipment, community inclusion, marriage, relationship building, assuring basic safety, coordinating health concerns, receiving all entitlements, developing hobbies, interest and talents, self advocacy and other areas that maximize a person’s quality of life.

Services

• New Opportunities Waiver (Provides services to people with developmental disabilities, ages 3 and older, who choose to remain living in the community.)
• Children’s Choice Waiver (Provides supplemental supports to children with developmental disabilities, birth through 18 years of age, who currently live at home or who will leave an institution and return home.)
• Supports Waiver (Provides employment services for individuals with developmental disabilities, age 18 or older. These services consist of supported employment, day habilitation, pre-vocational services, emergency respite, habilitation.)
• EPSDT Targeted Population medicaid Program (Coordinates and assists in accessing services for Medicaid eligible (OCDD) participants from the ages of three up to twenty-one on the DD request for Services Registry or who are determined to meet special needs are eligible.)
• Community Choices Waiver ( Provides services to elderly and disabled adults who meet admission requirements for a nursing facility and who can be served in the community.)
• Long Term- Personal Care Services (Provides assistance with activities of daily living for any qualified adult Medicaid recipient. Individual must meet the requirements for a nursing facility level of care.)
• Adult Day Health Care Waiver (Provides day activities to include the following:  assistance with activities of daily living, health and nutrition counseling, health education classes, hot meal and two snacks, social services, transportation, some health and nursing services, and exercise programs.)
