San Jose Clinic

The San Jose Clinic provides healthcare services exclusively to the low-income population.  The clinic offers primary care services for adults and children, women’s health services, dental care, vision care, counseling services, and a pharmacy. To receive services at the clinic individuals and families must first complete an eligibility interview.  Eligibility interviews are conducted on a walk-in basis from 8 am to 4 pm Monday through Friday.  No appointment is needed.  An eligibility application can be downloaded from the website-go to “Patient Center” then “Eligibility”.  A list of needed documentation is also on the website.
